Always in search of new ways to provide quality and complete passport services, Passport Canada now offers to Canadians an interactive form which can be filled on-line. This initiative, called passport on-line, is an addition to already existing PDF forms.

Different ways to access our passport application forms

Please note that all 21.59 cm X 35.56 cm/8.5 X 14 inch format (legal size) passport application forms (in Canada, in the United States and abroad) and all versions preceding (01-12) M02 for in Canada forms and (02-06) M02 for the United States and abroad forms are no longer accepted.

  Our passport application forms are available through our offices, at any Canada Post outlet and through Canadian Government Offices Abroad.

Replacement of a valid passport reported lost, stolen or damaged

When you apply for a new passport to replace a valid passport reported lost, stolen, damaged, destroyed or inacessible, you must include with the application form a completed "Statutory Declaration" form (PPT-203).
