Please complete this form to provide us with your best 14 weeks of insured earnings in the last 52 weeks of employment if you reside in one of the 23 regions designated in “Best 14 Weeks“ pilot project. Under this pilot project, the amount of your weekly Employment Insurance benefits will be based on your highest or best 14 weeks of insured earnings in the last 52 weeks – or since the start of your last claim. You can mail or bring this form to your local Service Canada Centre. Please ensure that you have answered all the questions and to date and sign the form on page 2. |
