Introduction to the passport on-line form

The passport on-line form is an interactive application that will guide you through all sections of the form. The advantage of this new initiative is to minimize errors and eliminate omissions. You will need to print your application form, sign it and have your guarantor sign your completed application form and one of your photos. You will have to either present your application with supporting documents to one of our points of service or mail it in.

The passport on-line form is not yet available to all Canadians.

If you can answer yes to all 3 questions below, you can use the passport on-line form to complete your passport application.

How does the passport on-line form work

There are 4 sections to the passport on-line form. Each part is sub-divided into a number of steps meant to help you successfully complete your passport application form.

  1. Completing the passport on-line form: Eight parts assist you in completing the Adult passport application form for Canadians living in Canada.
  2. Application requirements: A checklist of things to do once you have printed the application form.
  3. Submitting the application: A list of ways to present your passport application.
  4. Logoff: Reminders of things you can do to ensure your information is protected.

Using epass Canada

With epass login, you do not have to complete the entire form in one visit. When you save your application, you have 60 days to come back to complete it.

To register for an epass, you will need to create a username and password, and select and answer three questions. Registration for an epass may take up to ten minutes. To access passport on-line form anytime in the future, you will use the same username and password every time you logon.

If you already have an epass, you do not need to register again.

Filling-out a passport application using the passport on-line form

It will take approximately 20-40 minutes to complete the application using passport on-line form.

In order to complete all parts of the form, you will need the following:

  • Where you have lived during the past two years (addresses and dates).
  • Where you worked or studied during the past two years (including addresses and phone numbers).
  • Two personal references and an emergency contact person (including addresses and contact information).
