Reporting a Marine Occurrence

In accordance with mandatory reporting requirements under the provisions of the Canadian Transportation Accident Investigation and Safety Board Act, the Canada Shipping Act, and the Canada Labour Code Part II, where a reportable marine accident or incident takes place, the owner, operator, charterer, master, pilot or any crew member of a ship shall report to the Board as much information as possible by the quickest means available.
In addition to initial reporting requirements, the master of a ship involved in a reportable marine accident or incident shall, in a form approved by the Board (below), submit to the Board within 30 days after the accident or incident all information required. You can report a marine occurrence by completing a Form "Report of a Marine Occurrence / Hazardous Occurrence Report" and submitting it to TSB.

- Print the form and complete by hand (Please note that if you have Acrobat Reader® software, you can only view and print this Adobe PDF file)
- Mail a copy of the completed form to one of the addresses listed in the shaded portion of the form
