A Letter of Agreement has been prepared to document your school community’s commitment to the philosophy, principles and stages involved in participating in the Voices and Choices process. It describes the kinds of activities the school community will need to undertake during this process and addresses issues of confidentiality. It also contains an Acceptable Use Policy for using the on-line database. The success of your Voices and Choices project will depend to a certain extent on the level of agreement to these terms that you obtain from key players in your school community, including students, the school administration, parents, teachers and community partners. The Letter of Agreement must be signed by your school principal.
The School Demographics Questionnaire asks general questions about your school that will be used to set up a basic description for the on-line Database. This description will be kept confidential and will allow you to make anonymous comparisons to other similar schools (based on province, school size and community type). You are required to complete and fax back this questionnaire to the Canadian Association for Health, Physical Education, Recreation and Dance (CAHPERD) at (613) 523-1206 in order to obtain a school login and password for the Database.
The Student Needs Assessment Questionnaire was developed to gain a better understanding of student health-related needs and issues within the school setting. The questionnaire is based on the Voices and Choices principles and paths to school and student health and well-being. It is composed of 76 questions and takes about 50 to 60 minutes to complete. The questions examine how students feel about their health, what they would like to do to improve it, what's stopping them, and how the school can help. Many of the questions were drawn from the World Health Organization (WHO) cross-national Health Behaviours in School-Aged Children (HBSC) study.
