DRAFT - Instructions for Completing Each Item on the Movement Document

TABLE OF CONTENTS


Part A. Generator / Consignor OR Exporter / Foreign Exporter

  1. Identify the company name, the provincial identification number or the registration number, business (mailing) address, email address of and the telephone number for the generator / consignor OR exporter / foreign exporter and address from which the hazardous waste or hazardous recyclable material is shipped.

  2. Identify the company name, registration no. / provincial ID no., business (mailing) address, email address of and telephone number for the intended receiver / consignee OR importer / foreign receiver and the address of the intended site receiving the hazardous waste or hazardous recyclable material.

Description of hazardous waste or hazardous recyclable material

  1. Identify provincial code, if applicable, for each hazardous waste or hazardous recyclable material from the province or territory where the hazardous waste or hazardous recyclable material is being shipped.

  2. Identify for each hazardous waste or hazardous recyclable material, in accordance with the Transportation of Dangerous Goods Regulations (TDGR), the proper shipping name or description.

  3. Identify, in accordance with TDGR, the primary class and subsidiary class or classes for each hazardous waste or hazardous recyclable material listed.

  4. Identify, in accordance with TDGR, the UN number (TDGR Product Identification Number (PIN)) for each hazardous waste or hazardous recyclable material listed.

  5. Identify for each hazardous waste or hazardous recyclable material listed, in accordance with TDGR, the packing group code (I very dangerous; II dangerous; III moderately dangerous) or the risk group (IV most dangerous infectious substances; III infectious substances that are less dangerous than Group 1A; II infectious substances that are less dangerous than Group 1B) for each hazardous waste or hazardous recyclable material listed.

  6. Identify the quantity shipped and units in Kg or L of each hazardous waste or hazardous recyclable material listed.

  7. Identify the number of packages for each hazardous waste or hazardous recyclable material listed and the type of packaging (Int. Ext. Code) (01 drum; 02 tank; 03 bulk; 04 carton; 05 bag; 06 roll off or lugger; 07 other).

  8. Identify the physical state for each hazardous waste or hazardous recyclable material listed.

For international shipments, identify the LSPG Code. The LSPG code starts with "L" for liquids, "S" for solids, "P" for sludges and "G" for gases and includes a number which identifies the generic type of potential hazardous waste or hazardous recyclable material.

Information elements #11 to #19 apply to international shipments only

  1. Identify notice number and notice line number for each hazardous waste or hazardous recyclable material listed.

  2. Identify the shipment number and the total proposed number of shipments for each hazardous waste or hazardous recyclable material listed.

  3. Identify the disposal (D) or recycling (R) code which identify the operation to which each hazardous waste or hazardous recyclable material listed is intended.

  4. Identify the "C" code, which identifies that contaminants present in each hazardous waste or hazardous recyclable material listed

  5. Identify the Basel Convention on the Control of Transboundary Movements of Hazardous Wastes and Their Disposal (Basel) Annex VIII code as set out in List A of Annex VIII of the Convention or OECD Code as set out in Part II of Appendix 4 to the OECD Decision C(2001)107/Final (if different than Basel) for each hazardous waste and hazardous recyclable material listed.

  6. Identify the potential hazardous characteristic, "H" code, for each hazardous waste or hazardous recyclable material listed.Identify the category of the hazardous waste or hazardous recyclable materials, the "Y" Code, as set out in Annex I of the Basel Convention.

  7. Identify the national code in country of export or import for each hazardous waste or hazardous recyclable material listed.

  8. Identify the customs code from the Harmonized Commodity Description and Coding System prepared by the World Customs Organization, for each hazardous waste and hazardous recyclable material listed.

  9. The generator / consignor's OR exporter / foreign exporter's authorized representative shall print his/her name and telephone number, and sign the form certifying that the information given in Part A is correct and complete.

  10. Identify time and date that the hazardous waste or hazardous recyclable material are shipped and the scheduled arrival date.

  11. Identify special handling and emergency instructions. For example: keep in shade, drums are half full, keep away from water, etc., or in case of emergency or spill, immediately call the emergency phone number (listed on this form) for the province or territory in which the emergency or spill occurred, etc.

Part B. Carrier

  1. Identify the company name, the provincial identification number or the registration number, business (mailing) address, email address and the telephone number of the carrier.

  2. Identify vehicle trailer license number and province or territory of registration for each trailer or rail car being used to transport the hazardous waste or hazardous recyclable material.

  3. For international shipments, identify point of entry into Canada for imports or exit out of Canada in the case of exports. Transits will require both since there is both an entry into and an exit out of Canada.

  4. The carrier's authorized representative shall print his/her name and telephone number and sign the form certifying that the hazardous wastes or hazardous recyclable materials described in Part A have been received for delivery to the intended receiver / consignee OR importer / foreign receiver.

Part C. Receiver / Consignee OR Importer / Foreign Receiver

  1. If other movement document(s) or manifest(s) are used, identify reference number(s) of other movement document(s) or manifest(s) used.

  2. Identify the company name , the provincial identification number or the registration number, business (mailing) address, email address and telephone number of the receiver / consignee OR importer / foreign receiver and the address of the site receiving the hazardous waste or hazardous recyclable material, if different than identified in box 2 of Part A.

  3. Identify date and time that the shipment of hazardous waste or hazardous recyclable material is received at the receiving site.

  4. If the hazardous waste or hazardous recyclable material is to be transferred to another receiver / consignee, indicate company name and provincial identification number or registration number. (Not allowed under the EIHWHRMR. Need to meet rerouting or return provisions.)

NOTE: This option is not available in provinces or territories where the provincial or territorial legislation and regulations require the use of additional movement documents when more than one carrier is used for shipment into, out of or through their jurisdiction.

  1. Identify the quantity of each hazardous waste or hazardous recyclable material received and the units in kg of L.

  2. For domestic shipments,if the quantity of hazardous waste or hazardous recyclable materials or the number of small means of containment changes during transport, specify the reasons for the changes.

  3. Identify the final handling method for each hazardous waste or hazardous recyclable material by one of the following handling codes:
    • 01 storage
    • 02 thermal treatment
    • 03 chemical treatment
    • 04 physical treatment
    • 05 biological treatment
    • 06 secure landfill
    • 07 recycling
    • 08 solidification
    • 09 other [specify]

  4. For each hazardous waste or hazardous recyclable material listed, indicate whether or not the shipment was accepted or refused.

  5. Identify whether decontamination of packaging or the vehicle has been carried out for each hazardous waste or hazardous recyclable material listed, by writing "yes" or "no" in the appropriate column.

  6. If handling code is identified in box 33 is "other", specify how the hazardous waste or hazardous recyclable material is being handled.

  7. The receiver / consignee's OR the importer / foreign receiver's authorized representative shall print his/her name and telephone number and sign the form certifying that the information given in Part C is correct and complete.

Please also note the following:

  • Permits previously issued under the EIHWR will continue to be valid until the expiry date indicated on the permit.
  • Permits issued under the EIHWR do not contain a Basel Code, OECD Code, nor a Y code. Fill in "Not Applicable" or "N/A" for those fields in the movement document should they not be indicated on the corresponding Permit.
  • The D/R, C and H Codes, can be found in the International Waste Identification Code (IWIC) code describing the waste/material in the Permit.
