Health improvement networks are being formed throughout the region to help manage the care of people with chronic health conditions. These include:

• East Kootenay Health Improvement Network
• Integrated Health Clinic in Penticton
• Kamloops Health Improvement Network
• Kootenay Boundary Health Improvement Network (Nelson)
• Vernon Health Improvement Network

These networks are characterized by: a multiple disease orientation, an interdisciplinary team, and the provision of patient education while promoting self-management skills. Ultimately, the new and innovative programs and services offered through these networks will help people learn healthy ways to live with their chronic conditions … and enjoy a better quality of life!
