Solemn Declaration Concerning a Citizenship Certificate That Was Lost, Stolen, Destroyed or Never Received

This form is for Canadian citizens who:

  • applied for a citizenship certificate and six weeks have passed since the certificate was mailed to them. To know when your certificate was mailed, you can access your file on-line, or contact the CIC Call Centre.

or

  • possessed a Canadian citizenship certificate that was lost, stolen or destroyed but who do not wish to reapply for a new certificate immediately.

You may also choose to apply for a new citizenship certificate.
