Chapter 18 Lower Extremities

18.01 - Assessments for Lower Extremities

1. Assessments for loss, loss of the use of, or impairment of the lower extremities are shown in the Table to Article 18.01.

Table to Article 18.01
1. Loss of any toe, other than big toe 3%
2. Loss of big toe, with head of metatarsal 10%
3. Loss of foot:
(a) By midtarsal disarticulation (Chopart's Amputation) 30%
(b)Between metatarsus and tarsus (Lisfranc's Amputation) 30%
(c) Amputation of foot at ankle (Pirogoff's Amputation) 50%
(d) Disarticulation of foot with removal of both malleoli(Syme's Amputation) 50%
4. Loss of one leg at any level above Syme's Amputation (ankle disarticulation) to 4½" below knee 50%
5. Loss of leg below knee, less than 11.5 cm. stump 60%
6. Loss of leg through knee joint 60%
7. Loss of leg above the knee, lower 70%
8. Loss of leg, upper third thigh 80%
9. Loss of leg, upper third thigh 80%
10. Disarticulation of hip- joint 90%
11. Loss of both legs at any level, Syme's Amputation or above 100%
12. Hallux rigidus up to 15%
13. Bony fusion of ankle, optimum position 20%
14. Bony fusion of knee, optimum position 30%
15. Bony fusion of hip- joint, optimum position 50%
16. Injuries to nerves of leg, with complete loss of function.
(a) Sciatic 60%
(b) Lateral popliteal 20%
(c) Anterior tibial 10%
(d) Musculo- cutaneous 10%
(e) Medial popliteal 20%
(f) Post tibial 10%
(g) Femoral 50%
17. Shortening of leg
(2.5 to 7.5 cm.) 10%
(7.5 to 13 cm.) 15%
18.Osteoarthritis hip
mild 10- 25%
moderate 25- 35%
severe 35- 50%
19. Hallux valgus 0- 10% each foot
20. Osteoarthritis toes except great toe less than 3%
Osteoarthritis great toe 1- 5%
21.Pes planus or pes cavus in general Nil to 10%
If severe callosities, corns, may consider 15% to 20% rare
22. Osteoarthritis knee Nil to 40%
(a) full range, no effusion, stable, some crepitus 5- 10%
(b) lacks 10° extension minimum 15%
(c) flex only to 90° minimum 15%
(d) fused (optimal position) 30%
(e) unstable, lacks extension or flexes only 90° minimum 20%
(f) unstable, effusion, lacks extension or flexion beyond 90° minimum 20%
(g) unstable, effusion, lacks extension and flexes less than 90° 25%
(h) severely affected knee, unstable 30% to 40%
(i) prosthesis: ................ assess according to function
