You can view this document in the following format: Portable Document Format (PDF) minimum version 2.1. Or you can request a printed form or publication by phoning 1-800-267-7777 or by writing to us.

Note: Licence denials forms (L01-L02-L03) are strictly for the use of the provincial/territorial enforcement programs.

Fillible forms let you enter your data on-screen instead of completing forms by hand. After completing a form on-screen, you must print it and then submit the paper copy.

As the enforcement of family support obligations is a provincial responsibility, if you disagree with the financial terms and wish to make an arrangement or to dispute the garnishment of your federal funds, we would suggest that you contact the provincial or territorial agency.
