Statement of Appeal – Form 1

To file an appeal, a Statement of Appeal – Form 1 must be completed and addressed to the chief appeals officer and sent to the Commission via courier or registered mail along with the applicable fees to Chief Appeal Officer, Hazardous Materials Information Review Commission, 427 Laurier Avenue West, 7th floor, Ottawa, Ontario K1A 1M3. The Hazardous Materials Information Review Act Appeal Board Procedures Regulations will assist in completing this form.

Application – Form 2

If the appeal is concerning health and safety in the workplace, you have a choice to use either Part IX of the Statement of Appeal – Form 1 or the Application – Form 2. A Statement of Appeal – Form 1 must accompany the Application – Form 2.

Notice of Appeal – Form 3

For each appeal filed, a notice of appeal – Form 3 is published in the Canada Gazette to provide notice to those who intend to participate in the appeal.

Appearance – Form 4

A claimant or an affected party who intends to participate in the appeal, must complete an Appearance – Form 4.

Declaration and Undertaking – Form 5

If counsel to any party come within the limitations described in subsection 13(4) of the Hazardous Materials Information Review Act Appeal Board Procedures Regulations, he/she must complete the Declaration and Undertaking – Form 5.
