Application Form


Filing a Claim for Exemption Documents

To obtain a registry number, the information required by Section 8 of the Regulations Amending the Hazardous Materials Information Review Regulations must be submitted to the Commission along with the prescribed fee. The Commission has created a new document entitled Application for a Claim for Exemption to reflect the regulatory changes which came into effect on June 13, 2002. This application can be downloaded from the link below. The completed application should be addressed to the Client Services Division, Hazardous Materials Information Review Commission, 427 Laurier Avenue West, 7th floor, Ottawa, Ontario K1A 1M3, and sent via courier or registered mail along with the applicable fees or credit card authorization information.
