The following documents need to be attached to your application sent to the CGD, for temporary workers exemption:
- a copy of your passport (the pages with your photography and citizenship information only);
- a copy of your work permit issued by Citizenship and Immigration Canada;
- the original copy of your fingerprints (form C-216C, available at your local police station or with RCMP);
- a "certificate of good conduct" (criminal record check for the five last years, available at the embassy or consulates of your country in Canada): Optional (to prevent delay of fingerprints process that usually takes 6 months to a year).

- Application for Advisory Opinion (Form EXT 1790)
