Section Title: Applications and Forms

Application to Sponsor a Member of the Family Class

This application package is for Canadian citizens and permanent residents who wish to sponsor:

  • a spouse, common-law or conjugal partner; or
  • dependent children.

The package includes three parts:

  • Part A: The sponsorship application;
  • Part B: The application for permanent residence; and
  • Part C: The region specific forms.

What to do with the applications:

  1. Part A: You (the sponsor) must fill out the sponsorship forms. Use the checklist (form IMM 5491) to make sure you fill out all the right forms.
  2. Gather the documentation to support your sponsorship application.
  3. Part B: Have the person you are sponsoring, and any family members included in the application, fill out the immigration forms. You can send the forms to them or have them print the forms from this page.
  4. Part C: The person you are sponsoring must fill out the forms specific to their region. They should use the checklist from the information guide specific to their region (Appendix A) to make sure they fill out all the right forms. You can send the forms to them or have them print the forms from this page.
  5. After filling out the immigration and region specific forms, the person you are sponsoring must send them to you with the supporting documentation.

After you complete all the sponsorship forms and receive the completed immigration and region specific forms from the person you are sponsoring, you must send them to the Case Processing Centre. Make sure you send the sponsorship application, the immigration application and all supporting documentation at the same time.
