Appendix D
Medical Instructions/Designated Medical Practitioners (DMP)

The following instructions apply only to:

  • persons who are members of the family class, specifically the spouse or common-law partner of the sponsor, and whose application for permanent residence is processed at the Case Processing Centre in Vegreville.
  • their dependent children.

If you are not a person described above, do not use this form. If you do, the results of your medical examination will not be valid. You will have to undergo a new examination and pay the associated fees. If you are a citizen or permanent resident of Canada, you do not need to undergo a medical examination.

If you are a person described above, you and, if applicable, each of your dependent children must complete your own copy of the Medical Report: Section A - EDE form (IMM 1017 SCL). Make enough blank photocopies for your needs, before you start completing the form.

You must make an appointment for a medical exam with a Designated Medical Practitioner (DMP). When you go for your medical examination, take your copy of the Medical Report: Section A-EDE (IMM 1017 SCL) so that Section 2 can be completed by your doctor. The doctor will send the form with your results to the appropriate medical office for processing.
