Public Service Management Insurance Plan (PSMIP) - Change of Insurer - Information Notice

SUBJECT: Public Service Management Insurance Plan (PSMIP) - Change of Insurer

Purpose

The purpose of this notice is to advise you of a change in the insurer for the Public Service Management Insurance Plan (PSMIP).

Background

The Industrial Alliance Insurance and Financial Services Inc Company of Canada (National Life) has been the principal insurer for the PSMIP since its inception in 1968. National Life has been a subsidiary of Industrial Alliance Insurance and Financial Services Inc. (Industrial Alliance) since 1988.

Effective June 30, 2005 National Life's operations were transferred to Industrial Alliance. This transfer represents the culmination of the harmonization of business activities between the two companies.

As a result of this change, Industrial Alliance has assumed National Life's contractual commitments, including all duties, obligations and liabilities with respect to the PSMIP Group policy G68-1400.

THIS CHANGE DOES NOT AFFECT PLAN MEMBERS OR THEIR BENEFITS.

It is the responsibility of each department to advise employees of this change and to ensure they understand that this change in no way affects their coverage or benefits. Would you ensure that the attached NOTICE TO EMPLOYEES is distributed within your organization.

Details

There will be no change in the administration of the plan. The mailing address for the insurance company and the insurance team for the plan will remain the same. However, forms and publications are being updated to change the name of the insurer.

Compensation officers should consult the appropriate websites for electronic versions of the revised forms.

The address and process for ordering copies of printed forms is unchanged.

The insurer has already mailed a small supply of printed claims forms to departments and employers.

Departments are requested to begin using the new forms. Existing stock of the old forms should be destroyed.

There is one exception. Revised Declaration of Health forms (2028-2 and 2028-3) are not yet available but will be available soon. The insurer will continue to process the current Declaration of Health forms until the revised forms are available.

NOTICE TO EMPLOYEES

SUBJECT: Change of Insurer for Public Service Management Insurance Plan (PSMIP)

The Industrial Alliance Insurance and Financial Services Inc Company of Canada (National Life) has been the principal insurer for the Public Service Management Insurance Plan (PSMIP) since the inception of the plan in 1968. Effective June 30, 2005 National Life's business operations were transferred to its parent company, Industrial Alliance Insurance and Financial Services Inc.(Industrial Alliance) and, as a result, Industrial Alliance will be the insurer for the PSMIP.

This change will not affect the terms of your insurance in any way. The PSMIP Group Policy G68-1400 was transferred to Industrial Alliance. The transfer does not result in any change to the policy, and Industrial Alliance will assume all duties, obligations and liabilities in connection with the policy.

In addition, there will be no change in the administration of the plan, except for the change in the name of the insurer on the forms and publications. The mailing address for the insurance company and the insurance team for the plan will remain the same.

National Life has been a subsidiary of Industrial Alliance since 1988 and this transfer represents the completion of the harmonization of business activities that has been underway for several years.

The transfer of National Life's business operations to Industrial Alliance has been approved by the Minister of Finance.
