Disclosure Statement Forms

Below are the various forms that Members are required to complete and file with the Office of the Ethics Commissioner.

  • Disclosure Statement for Members and their Family Members PDF is the form which members need to complete in order to disclose their private interests and those of their family members. The Code stipulates that this statement is to be filed within 60 days of their election appearing in the Canada Gazette.
  • Public Statement of Sponsored Travel by Members PDF must be completed and filed within 30 days after the end of the trip whenever a trip that arises from or relates to the Member's position exceeds $500 and is not wholly paid from the Consolidated Revenue Fund or by the Member personally, his or her political party or any interparliamentary association or friendship group recognized by the House.
  • Statement of Gifts or Other Benefits PDF must be completed and filed within 30 days after receiving the gifts or other benefits, when gifts or other benefits which are acceptable under the Code exceed $500 in value, or if the total value of all such gifts or benefits received from one source in a 12-month period exceeds $500. After receiving this form, the Office of the Ethics Commissioner will prepare a Public Statement for the Member's signature, which will thereafter be added to the Public Registry.

    Note: Although gifts or other benefits may not exceed $500 in value, whether they are acceptable or not under the Code must also be determined. Members should contact the Office of the Ethics Commissioner to find out if a gift or another benefit is acceptable.
