Disclosure Requirement Forms for Public Office Holders
Ministers and Parliamentary Secretaries who are also Members of the House of Commons, only need to complete the Supplementary Disclosure Statement, in addition to their Member’s Disclosure Statement, in order to meet the confidential disclosure requirements of section 9 of the Conflict of Interest and Post-Employment Code for Public Office Holders. All other Public Office Holders must complete the Confidential Report provided below.
