What is a T4E?

A T4E form is issued to everyone who received EI benefits or repaid an overpayment in the previous year, for example, from January 1st, 2005 to December 31st, 2005. This form must be filed with your income tax return for that tax year. The T4E indicates the gross amount of benefits paid, the income tax deducted and the amount paid toward an overpayment (if applicable).

All T4Es for taxation year 2005 will be issued by February 28, 2006. If you do not receive your T4E by March 3, 2006, please call our telephone information service at 1 800 206-7218 from 8:30 am to 4:30 pm or go to your Service Canada Centre and we will issue a duplicate T4E.

What is an Itemized Statement of Benefits Paid?

An Itemized Statement of Benefits Paid is a breakdown of EI benefits received during a calendar year. You can get an Itemized Statement for an EI claim for up to the last 7 years.

When you receive your T4E, if you disagree with the information on it or would like additional information, you may request an Itemized Statement.

How to order a duplicate T4E and/or Itemized Statement

You can make your request in writing. To do so, please print and complete the form Request for duplicate T4E and/or Itemized Statement of Benefits Paid and mail it to the Tax Enquiry Control Unit. You must submit a photocopy of proof of identity with your request.

Examples of proof of identify

  • Social Insurance Number  (SIN) card
  • Health Insurance card
  • Driver's license

You can also make your request by calling our telephone information service at 1 800 206-7218 from 8:30 am to 4:30 pm or in person to your Service Canada Centre. A representative will:

  • verify your SIN, name, current address and access code;
  • perform a detailed ID check, if your access code is unknown;
  • request the Itemized Statement from the Tax Enquiry Control Unit related to taxation year you require.

Representatives from the EI Call Center and local offices can issue a duplicate T4E. Allow 4 weeks for processing and delivering from the date of your request for an Itemized Statement.
