Request to Examine Personal Information

The Privacy Act Government of Canada site gives Canadian citizens and people present in Canada the right to have access to information that is held about them by the federal government. This means you have access to your Social Insurance Number (SIN) record contained in the Social Insurance Register. The type of information retained in the register is defined in bank number.

To request copies of your previously submitted SIN application forms, you must complete a Personal Information Request form (TB35058) Government of Canada site or a Social Insurance Number (SIN) application form (NAS 2120) and indicate on the form that you are requesting the copies under the Privacy Act, Bank HRDC PPU 390.

The form must be accompanied by documents which prove your identity and status in Canada.

The rights or actions provided for under the Privacy Act and the Regulations with respect to privacy may be exercised or performed:

  1. on behalf of a minor or an incompetent person by a person authorized by or pursuant to the law of Canada or a province to administer the affairs or estate of that person;

  2. on behalf of a deceased person by a person authorized by or pursuant to the law of Canada or a province to administer the estate of that person, but only for the purpose of such administration; and

  3. on behalf of any other individual by any person authorized in writing to do so by the individual.
