The English versions are intended solely to help you fill out the French forms. The translations are not the forms to fill in.

Downloading the forms
All
our forms are available as dynamic PDF files, meaning you can fill them out
directly on-screen.
You
must use Adobe Reader 5.0 (or higher), which is available free of charge
from the Adobe website. The free version of Acrobat Reader, however, does not
let you save the information entered.
Printing the forms
Please print the forms with black ink on white paper to ensure superior quality when they are microfilmed.
To obtain the required number of copies, it is important to press the "print" button found on the last page of the form. All copies must be signed by hand.
Submitting the forms
Since at present the forms cannot be submitted electronically, after duly completing and signing them, you should bring them, along with the specified fees, directly to a REQ service counter in Québec City or Montréal, or mail them to:
