This is a quick, convenient and secure way to notify the Ministry of Transportation, the Ministry of Natural Resources and the Ministry of Health and Long-Term Care of your change of address. Please do not change your address until you move or until it becomes effective.

Using our online form to change your information, you can choose to notify one or more of the three ministries of your change in one easy process. Your personal information will not be shared among the ministries. Each ministry will only get the information you send to them. Your privacy will be protected as required by the Freedom of Information and Protection of Privacy Act and the Personal Health Information Protection Act.

Before you begin: 

• If you experience difficulties while using the ServiceOntario address change, please refer to the Frequently Asked Questions or get information on other ways to change your address(Question 34).
• The Ontario government is committed to ensuring the privacy of your personal information. Please note that some non-standard browsers may retain/cache your personal information. To prevent this, always use the "Next" and "Previous" buttons on the pages, and close your browser to end the session.

Minimum Windows requirements:

• Internet Explorer 5.1 or higher
• Netscape 6.x or higher
• Mozilla Firefox 1.0 or higher

You can visit the Microsoft Internet Explorer website or the Netscape Navigator website to upgrade your browser for free.

Minimum Mac requirements:

• Mac OS 10.3.9 with Internet Explorer 5.1 or higher
• Mac OS 10.3.9 with Safari 1.1 or higher

Minimum Linux requirements:

• LinuxRedhat 2.4.21-4EL with Mozilla 1.4 or higher
• SuSELInux 2.4.19-4GB with Mozilla 1.2.1 or higher 

• You may need to increase your screen resolution (for example to 1024 x 768 pixels) to ensure screens are properly displayed.

Here is what you will need to get started:

• Your complete NEW residential address (where you live).
• Your complete NEW mailing address (where you receive your mail if different than where you live).

• Your Driver's Licence Number from your Driver's Licence;
  OR
• Your Registrant Identification Number (RIN) from your Vehicle Registration;
• Your postal code last reported to the Ministry of Transportation;
• Your complete NEW residential address (where you live);
• Your complete NEW mailing address (where you receive your mail if different than where you live).

• Your Outdoors Card Number;
• Your date of birth;
• Your complete NEW mailing address (where you receive your mail).

If you have a red and white health card you will require:

• Health Number;
• Version Code;
• Your postal code last reported to the Ministry of Health and Long-Term Care;
• Your complete NEW residential address (if different than mailing address). 
  
  

If you have a photo health card you will require:

• Health Number;
• Version Code;
• Canadian citizenship or immigration document number you used when registering for your photo health card;
• Your complete NEW residential address (if different than mailing address).
