Your Community Health Services

Community Care Access Centres
There are 42 Community Care Access Centres (CCACs) in Ontario, two of which are hospital-based. CCACs provide a simplified point of access to long-term care for more than 400,000 people each year.

Community Health Centres
Community Health Centres (CHCs) are non-profit organizations that provide primary health and health promotion programs for individuals, families and communities. A health centre is established and governed by a community-elected board of directors.

General Hospitals
There are 211 hospital sites in Ontario of four different types, including Public Hospitals, Private Hospitals, Federal Hospitals, and Cancer Care Ontario Hospitals. This page provides information on hospital locations and classifications.

Seniors' Care : Home, Community and Residential Care Services for Seniors
Care programs provide a variety of health-related services that help to support people at home or in facility-based care.

Ontario Health Insurance Offices (OHIP)
A resident of Ontario must have a health card to show that he or she is entitled to health care services paid for by the government. This page shows the locations of OHIP offices in your community. See the Ontario Health Insurance pages for detailed information on the program.

Psychiatric Hospitals
This page provides a list of hospitals in your community that provide mental health services.

Public Health Laboratories
This is a location list of regional public health laboratories.

Public Health Units
Health units administer health promotion and disease prevention programs to inform the public about healthy life-styles, communicable disease control including education in STDs/AIDS, immunization, food premises inspection, healthy growth and development including parenting education, health education for all age groups and selected screening services.

Service Ontario Kiosks
ServiceOntario is a network of automated self-service kiosks where you can change your address for your Ontario Health Card free of charge. This page lists the locations of kiosks in government offices and major shopping centres across the province.
