Public Reporting on Long-Term Care Homes

This online reporting tool will provide you with information about the performance of long-term care homes during a specified reporting period. The information will be updated regularly.

The provision of this information does not imply any endorsement or recommendation on behalf of the Province of Ontario, of any long-term care homes shown here.

You can use this tool to see different types of information about a home :
  • Find the location of any home in Ontario and some basic information about it, such as the address and number of beds.
  • See the Inspection Findings — reports issued by the Ministry of Health and Long-Term Care — on any home.
  • Read the Verified Concerns about a home, if any exist, such as complaints from residents or relatives.
  • Compare Inspection Findings and Verified Concerns, side-by-side, for up to five homes at a time.

You will not be able to apply directly for a long-term care home from here. Please contact your local Community Care Access Centre. CCACs authorize admissions to homes.

To get a feel about culture, on-going activities and general atmosphere of a home, you will have to visit homes that interest you. Use a checklist of things to consider and speak with the staff and residents.
