Based Scholarship Verification: |

from applicants in receipt of scholarships
received over more than one semester.

Work Term Income Confirmation: |
Required from applicants in receipt of income during a work
term, verified by the employer or educational
institution.

Permanent Resident Card: |
Required from Permanent Residents to confirm sponsorship.

Protected Persons Status Document (PPSD): |
Must be valid for the entire period of study; apply online at

Series Social Insurance Card: |
Application
can be made for temporary SIN cards online at
www.servicecanada.gc.ca.

Common-law Applicants: |
Common-law
applicants must provide a letter from an independent
third party that verifies the applicant’s living
arrangements for the 12 months prior to the start of the
study period. Letters from landlords and affidavits are
not acceptable.

Information/Signature - one parent: |
While information from both biological parents is required,
information from one parent may be accepted in cases where there are
legitimate exceptional circumstances, provided a satisfactory
written explanation by the custodial parent is submitted to the
Student Financial Services Division.

Disabled Dependents: |
Applicants
with a disabled dependent(s) over 11 years of age, must
submit a letter from a physician confirming that daily
care is required, and also provide receipts verifying
the amount claimed.
|
Foster
Children: |
Applicants
reporting a foster child(ren) as a dependent must
provide a letter from the appropriate government agency
verifying the amount of money being received to support
the child(ren).
