Notification FormNotification Process for Operational StatementsIf you determine that your project meets the requirements outlined in one of the Operational Statements, we ask that you notify DFO before starting your work by filling out and sending in, by mail or by fax, a Notification Form to your local DFO Office, preferably 10 business days (14 days in Alberta) in advance. This information is being requested as part of DFO's monitoring initiative, which will evaluate the effectiveness of the Operational Statements and allow DFO to make improvements or adjustments to the Operational Statements as necessary. Notification Forms for the provinces and territories of the Central and Arctic Region are listed below. Please choose the Notification Form for the province or territory where the project is located.
