Home Care

The Yukon Home Care Program provides health services to individuals who cannot easily access health care in the community, or who need assistance in performing daily activities. The program aims to improve people’s quality of life and supports them to live independently in their own homes.

Eligibility

To be eligible for services offered by the Yukon Home Care Program, a person must have a valid Yukon Health Care Insurance number.

Types of Service

The Yukon Home Care Program provides long-term, acute, palliative and respite care to Yukoners of all ages. Services are based on an individual’s assessed need and the amount of family and community support available to them. Descriptions of the services and the staff who provide them are listed below.

Services in Whitehorse

Social Workers

Meet with people referred to the Program and assist with identifying client needs and supports. They provide advocacy, counseling and support in accessing community resources.

Home Support Workers

Provide assistance to people needing help with activities of daily living. They provide personal care, home making and respite services. These services include assistance with bathing, grooming, light housekeeping, laundry, grocery shopping and meal preparation.

Occupational Therapists

Assist individuals to do the things important to them in day-to-day living. This may range from getting dressed to pursing a hobby. They may also recommend personal aids or home modifications.

Physiotherapists

Provide therapy services to people with physical problems in order to promote mobility, independence and physical fitness

Registered nurses

Provide assessments, treatments, teach clinical techniques to support self-care, consult with the medical community, and refer clients to other health providers. Examples of care provided are palliative care, wound care and ostomy care.

Services available outside of Whitehorse

The Yukon Home Care Program has home support workers who provide personal care, home making and respite services to communities outside of Whitehorse. An exception is Dawson City, where home care is provided through McDonald Lodge.

Additional services are provided through the Regional Services Office, the Community Health Centres and Regional Therapy Services.

You may refer yourself to the Yukon Home Care Program or have someone call on your behalf. To make a referral, call the telephone number listed for your community.
