How to order a Yukon birth certificate

On application, any person furnishing information satisfactory to the registrar and paying the prescribed fee, may, if the registrar is satisfied that it is not to be used for an unlawful or improper purpose, obtain a certificate in the prescribed form, ie:

• The person whose birth is recorded;
• Individuals recorded as parents on the birth record;
• An agent, or any other person, on the written authorization of the person named above;
• Guardian (copy of court recognized legal guardianship must be provided);
• The executor of the person's estate (copy of death certificate must be included with the application).

Certificate Types

• Wallet (small certificate) - size is 9.5 cm x 6.4 cm (2.5"X3.75")
  Contains the individuals full name, sex, date of birth, place of birth, registration number, and date of registration.
• Framing (large certificate) - size is 21.6 cm x 17.8 cm (7"X8.5")
  Contains the individuals full name, sex, date of birth, place of birth, registration number, and date of registration as well as the parents full names and their province/state or country of birth.

How to Order a Certificate or a Birth Registration

• Apply in person at any Vital Statistics Office or Territorial Agent
• Apply by mailing a completed Application for Service form. This application must be submitted to Vital Statistics with the appropriate fee payable by cheque, money order or credit card payable to Government of Yukon.
• Apply by phone (867-667-5207), paying by VISA, Mastercard, or American Express.

How to register a birth in Yukon

1. The birth of every child in the Territory must be registered within thirty days after the birth.
2. The responsibility for registering the birth rests with:
1. The mother and the father.
2. Where the father is unacknowledged, or either parent is incapable, by the other parent.
3. Where both parents are incapable, by the person standing in place of the parents.
3. No birth may be registered containing more than two surnames hyphenated or combined. If either the mother or father, or both, have a hyphenated or combined surname only one of the names in their surname or surnames may be used. In this case, the one name used must be the name that alphabetically precedes the other.
4. The information on this form is collected under the authority of the Vital Statistics Act. The information provided will be used to register this birth, produce birth certificates and provide statistical and demographic information.
