DOCUMENT CHECK LIST New York, NY FOR A CANADIAN TEMPORARY RESIDENT VISA The following documents are required to complete your application. If you are missing documents on this list, your application may be refused. All documents in a language other than French or English must be officially translated. Provide both a photocopy of the document and the translation in English or French. I have enclosed the following items: Put a "v" in the box when you have enclosed the item. 1. COMPLETED application form. Please complete the fillable applicaton form and print it for submission to our office. For ease and speed of processing, we ask that you do not complete the application form by hand. If more than 3 dependents travelling to Canada, please complete a second application form to include the additional dependents. 2. COMPLETED supplementary information sheet 3. Resumé/C.V. (Curriculum Vitaé) and additional questionnaires for nationals of any of the following countries: For ease and speed of processing, we ask that you print clearly your answers using CAPITAL letters in each box. If a question does not apply to you, write N/A meaning "not applicable" in the space provided. 4. Original valid passport valid for at least 6 months and with at least one blank page 5. All US immigration documents (original I-94, endorsed I-20, I-797, DS2019, advance parole letter...) you've held during your stay in the United States. 6. Processing fee: cash, certified cheque, or money order. We DO NOT accept payment in CASH when application is submitted by mail. We accept payments by Certified Cheque or Money Order for applications submitted in person and/or by mail.
7. Two (2) identical passport photos taken within the last 6 months (name and date of birth written on the back). Click here for photo specifications. If photographs do not meet these specifications, no file will be created and the application will be returned to the applicant. 8. For your common-law partner, statutory declaration of common-law union (IMM5409). 9. Proof of employment in your country of residence. (letter from employer, pay stubs) 10. Proof of funds (i.e. bank statements). 11. Documents relating to your trip, such as an invitation from business or personal contacts in Canada, a letter from your employer, registration for a conference, your itinerary, appointment letter from USA Consulate, etc 12. Previously held passports showing your travel history 13. If on B1/B2 status: photocopy of your valid return ticket to your home country. 14. For TRANSIT VISAS, photocopy of your airline ticket with confirmed flight bookings. 15. For minors travelling with only 1 parent/no parents; notarised consent letter from parent(s) not accompanying & birth certificate of the minor child. 16. A complete itinerary, cities you intend to visit, full names, addresses and telephone numbers of all personal and/or businesses you will contact and your method of travel.
17. For diplomatic/official passport holders, note-verbale from your office stating the purpose of travel to Canada.
