Instructions for Downloading and Submitting Application Forms (HTML)

Downloading an Application Form

  • Select appropriate application form.
  • Go to "File" and select "Save As". Save the application form to your personal computer.
  • To fill out the form, right click on the document you saved on your computer and select a word processor from the menu.
  • Complete the form and save.

File Folder

When you download an application form, you will see a file folder as well as the document. There is no need to open this folder: it contains two images that must appear on the electronic form (that is, the CRTC logo and the Canada wordmark).

Submitting your Application Brief

The Application Brief includes your completed application form along with the Book of Supporting Documents, identified as Appendices in the form.

Submit your Application Brief in the original format – that is, HTML. See bullet four in Downloading an Application Form.

Submit your application electronically by attaching it to the Cover Page, which is available on our web site under "File, Register and Epass". One hard copy version must also be sent to the Secretary General, CRTC, Ottawa, Ontario K1A 0N2. The onus is on the applicant/licensee to ensure that the electronic version is identical to the hard copy version.
