In order to reduce the reporting burden on generally smaller entities, the CRTC has taken steps to stratify the industry into two groups (Group1 and Group2). Your organization has been identified as a member of the Group 2 reporting entities, required only to complete a simplified Registration Form package. Detail of this process and the criteria for selection can be found in Telecom Circular 2005-4, Ottawa, 9 February 2005. Enclosed in the Simplified Registration Form package is: Privacy Notice – this document explains the purpose and use of certain personal information that is collected as part of the Registration update process; Parts A, B, C and D of the Simplified Registration Form – Filed in Confidence, period ending 31 December 2005 – this form contains pre-populated information about your entity information, related to monitoring activities; and Parts E and F of the Registration Lists Contact Information – this form contains pre-populated information about your current Registration Lists and the Contact Person Information associated with each Registration List. Please complete and submit the attached forms to the CRTC using the enclosed stamped self-addressed envelope by 1 March 2006.
