dated 7 April 2003 and amended on 14 May 2003, in order to increase its
monthly rates as follows: residential individual line from $22.30 to $22.84;
residential line Touch Tone from $2.55 to $2.60; business individual line
from $46.55 to $47.55; Centrex access from $45.45 to $46.45; and Centrex
