CRTC Telecommunications Industry Data Collection
Forms - 2004
As the Reporting
Guide explains, this year's data collection consists of
a two-step process. The first step involves issuing the Reporting
Entity Profile (REP) form on 6 February 2004 . Information
collected from this form will be used to identify which particular
Data Forms, covering specific CRTC process information, a reporting
entity will have to complete in the second step. The Data Forms
will be issued at the beginning of March 2004 with varying
completion dates depending on the particular Commission activity
in question. The filing dates are:

the Governor in Council (Monitoring Report)

Blank REP forms may be viewed below and the Data Forms will
be viewable when they are released at the beginning of March
2004. These forms are for information only; official submissions
are to be made via the Commission's new web-bases Data Collection
System.
If you are an entity involved in one or more of the Commission
processes mentioned in the Telecom Circular, and have not yet
nominated a Response Manager, rather than attempting to print
off the forms on this page and fill them in, please first nominate
a Response Manager by following the correct procedure in Telecom
Circular 2003-1.
These forms will be posted at the beginning of March.
