CRTC Telecommunications Industry Data Collection
Forms - 2005
For the 2005 data collection the industry has been stratified into two groups full details of which can be found in Telecom Circular 2005-4. Entities in Group 1 will continue to use the web-based Data Collection System (DCS) while entities in Group 2 will be required to fill in a pre-populated simplified REP form that will be mailed to them.
If you are an entity involved in one or more of the Commission processes mentioned in Telecom Circular 2003-1, and have not yet nominated a Response Manager, rather than attempting to print off the forms on this page and fill them in, please first nominate a Response Manager by following the correct procedure in Telecom Circular 2003-1 .
Group 1
As the Reporting Guide explains, this year's data collection consists of a two-step process. The first step involves issuing the Reporting Entity Profile (REP) form on 18 February 2005 . Information collected from this form will be used to identify which particular Data Forms, covering specific CRTC process information, a reporting entity will have to complete in the second step. The Data Forms will be issued once the completed REP form has been submitted to the Commission with varying completion dates depending on the particular Commission activity in question.
