Identification

The 2004 Telecommunications industry data collection consists of a two step process. The first step involves the issuance of the Reporting Entity Profile Forms on 6 February 2004 , while the second step is the issuance of Data Forms at the beginning of March 2004 .

The Reporting Entity Profile Forms are to be completed by each legal entity that supplied one of several telecommunications-related services, products, and/or facilities in Canada on a commercial basis at any point during 2003. These services and products are identified and detailed in the Reporting Entity Profile (REP) form .

The responses to the REP form will be used to determine which Data Forms each entity will be required to complete. Each reporting entity will be notified by e-mail and through their Response Manager which specific Data Forms they are to complete. Persuant to Telecom Circular 2003-1, Ottawa, 11 December 2003, and the Overview , each entity affected by one of the following CRTC activities: annual Monitoring, updating telecommunications entity registration lists, International licences, Telecommunications fees, and the contribution regime, was to appoint a Response Manager by 14 January 2004.

A separate set of forms should be filed for each legal entity; reporting may not be consolidated across companies, even where one company holds a majority stake in another. Where the legal entity in existence on 31 December 2003 is partly formed of merged companies, survey responses should include combined data for all predecessor companies for the entire year.

Guide to Forms

The Forms for are divided into two sets. The first set, Reporting Entity Profile (REP) form, is to be submitted by 23 February 2004 . All reporting entities are required to submit this form. The second set is the Data Forms that each entity is required to submit will depend upon the responses submitted on the REP form.

The Data Forms are due to be issued at the beginning of March 2004 with varying completion dates depending on the particular Commission activity in question. The filing dates are:

  • Financial statement forms
31 March 2004
  • Telecommunications fees
31 March 2004
  • Contribution regime
31 March 2004
  • Registration List Contact Information
30 April 2004
  • Data for the annual preparation of the Report to
    the Governor in Council (Monitoring Report)
30 April 2004
  • International licence renewals
30 April 2004

Reporting Entity Profile (REP) form: To be returned by 23 February 2004 . These forms have several broad purposes. They establish each reporting entity's contact details and corporate ties to other reporting entities, they provide a basic statement of telecommunications activity areas, and they update contact details and reporting responsibility.

Data Forms: These forms are divided into five CRTC activities: Monitoring of the Canadian telecommunications industry, international licences, Canadian contribution mechanism fund administration, telecommunications fees and CRTC registration lists. Each of these activities consists of a series of one or more forms. The form numbering system for these activities is:

1XX Financial
2XX Monitoring
50X International Licences
60X Canadian Contribution Mechanism Fund Administration
70X Telecommunications fees
80X CRTC Registration Lists

Completed forms should be submitted by the Response Manager using the Commission's new web-based Data Collections System.

Instructions

Legal Entities

Separate surveys should be filed for each legal entity; reporting may not be consolidated across companies, even where one company holds a majority stake in another. Where the legal entity in existence on 31 December 2003 is partly formed of merged companies, responses should include combined data for all predecessor companies for the entire year.

Reporting Period

Complete each form for year-ended 31 December 2003 except where otherwise noted. Information should be provided for the reporting entity, including any companies merged with it prior to 31 December 2003 . Note that separate reporting is required for any acquired entity that is still operating as a legal entity on 31 December 2003 .

Due date

The Reporting Entity Profile (REP) form is to be completed by 23 February 2004 while the Data Forms have varying completion dates depending on the particular Commission activity in question. The filing dates are:

  • Financial statement forms

  • Contribution regime

Submission

Completed forms should be submitted by the Response Manager using the Commission's new web-based Data Collections System.

Form Integrity

Each form includes shaded and non-shaded areas. You will only be able to enter data into the shaded areas.

Coordination

Please ensure that all definitions and interpretations correspond to published guidelines. All guidelines, forms, glossary, and other instructions are available for download at the introduction page. Estimates

For the Monitoring forms only, where precise data are not available or cannot be obtained, use best estimates. If estimates are used, describe the estimation methodology, giving the relevant form reference cell numbers, in an e-mail to the appropriate CRTC analyst (contact).

Revenues

Report revenues net of discounts, returns or allowances, promotional offers, and rebates. These revenues should exclude federal or provincial taxes collected for remittance to a government agency (sales and excise tax, GST, PST, HST, etc.). Goods and services sales denominated in foreign currencies should be translated into Canadian dollars in accordance with Section 1650 of the Canadian Institute of Chartered Accountants Handbook.

Scalors

Dollar amounts should be reported in units. Bandwidth amounts should be reported in millions of bits per second (Mbps). Traffic (minutes), lines, subscriptions, and accounts should be reported in units. All other measures should be reported in unscaled units, except where otherwise specified. For example, dollars are to be reported in whole dollars, minutes in whole minutes, etc.
