The Reporting Entity Profile (REP) form are to be completed by each legal entity that supplied one of several telecommunications-related services, products, and/or facilities in Canada on a commercial basis at any point during 2003. These services and products are identified and detailed in the services section of the REP form. Responses to the REP form will be used to determine which completed Data Forms will be required from each entity. Confidentiality: Is any of the information provided in this Reporting Entity Profile form confidential pursuant to section 39 of the Telecommunications Act? If so check the box and provide your claim for confidentiality in accordance with section 19 of the CRTC Telecommunications Rules of Procedure in the Box.
