Veterans Affairs Canada (VAC) can help you with outpatient follow-up and discharge planning.
Clients Eligible for Services From VAC:
- Veterans;
- Canadian Force (CF) members; and
- Serving or former members of the RCMP.
VAC Services:
Depending on each client's eligibility, VAC offers a range of services and benefits including:
- specialized services for clients with operational stress injuries (OSI) like depression, anxiety, or post-traumatic stress disorder (PTSD);
- health benefits such as medical, surgical, dental care, prescription drugs, hearing and vision aids and psychological counselling;
- the Veterans Independence Program, a national home care program providing support for grounds maintenance, housekeeping, personal care and certain changes to the home;
- long-term care for our older clients;
- case management; and
- a number of other health benefits, programs and services.
Please involve us early in discharge planning and outpatient follow-up as we don't provide emergency service 24 hours a day, seven days a week. Without prior notification, we cannot guarantee services and supports will be in place if a client is discharged on the weekend.
