VAC ON BASE

As part of a pilot project, the Department has placed Transition Coordinators at seven Canadian Forces locations to help CF members and their families who require additional support when releasing from the Forces. This may be particularly important for members who are leaving for medical reasons.

Our Transition Coordinators will work one-on-one with you to let you know about VAC programs and services to which you may be entitled and to help you make the transition from military to civilian life. VAC's Transition Coordinators complement the work being done by the DND's Case Managers.
