Please supply following information: 

• Your name, phone & fax numbers, and e-mail address.

• Company name and full address.

• Video: titles.

•  Date: each is to be picked-up, or sent out.

• Return date of video(s) - they must be at WSH Division office by this date.

Videos are loaned free of charge, BUT borrower is responsible for shipping and receiving. 

Maximum of four videos is normally booked per borrower at one time.

Videos must be returned to Workplace Safety & Health Division by the due date.

For   Winnipeg  Borrowers: - videos are normally booked for seven calendar days per borrower.

For Borrowers – outside the city: - videos are booked for 10 to 14 calendar days.

• they will be sent out by bus collect.

• Videos must be returned by bus, prepaid, with door to door service, by the due date.

*** Borrowers will be held responsible for the replacement cost of any lost or damaged videos.
