Health Care Card

Each NWT resident enrolled with the NWT Health Care Plan  is given a Health Care Card with a unique lifetime identifier for health care called a personal health number. This number remains the same, regardless of any changes to personal status. Keep this card with you as you must present the card to receive health care benefits. However, if you do not have your card with you, your physician or other health care provider can verify that you are enrolled with the NWT Health Care Plan.  

To ensure your coverage is maintained, notify the Department of Health and Social Services if: 

  • you lose your card; 

  • your name or address changes;

  • you move out of the NWT; or

  • if you are outside of the NWT for more than three months due to medical reasons, studies, or work. 

Note: You need to fill out a temporary absence form if you are outside of the NWT for more than 3 months.

You can use a Change of Address, Change of Name or Change of Aboriginal Status form if your personal information has changed.

Please note that Health Care cards are printed every Friday.
