CCAP is the NWT assessment tool used to conduct a needs assessment of the client and identify the appropriate continuing care services for that individual within the NWT continuum of care. The CCAP package includes all the forms and information that are part of this assessment tool. The CCAP Handbook provides the user with a guide to the package.
