The Canadian Best Practices Portal for Health Promotion and Chronic Disease Prevention is a central and early component of the Canadian Best Practices System. The Portal provides decision makers with easier and informed access to published and unpublished sources related to effective and emerging best practices approaches (models and theories), methods, research evidence and practical experience.

Integrated approach to chronic disease
Noncommunicable diseases accounts for most of the causes of death and disease burden in Canada and globally. No one entity can reduce noncommunicable disease alone. Thus an integrated approach to noncommunicable disease is critical to augment and enhance disease specific strategies.

Surveillance
Surveillance is an essential component for the planning, implementation, and assessment of chronic disease prevention and control. It is an ongoing collection of data, the integration, analysis, and interpretation of data, and the dissemination of information to those who need to know. Chronic disease surveillance provides relevant information for action.
