Canadian businesses

Please note that since not all offices offer this service in person, we advise that you call first.

You can order the ROE form : 

  • by telephone: To obtain the number, select your region under "Services where you live" and go to "Local Offices"; or
  • by completing the ROE order form and faxing or mailing it to the office nearest you.

When placing your order, please forecast your ROE forms requirements for a 6 month period.

Non-Canadian businesses operating in Canada

If you are a non-Canadian employer who wants to order ROE forms, your place of business registered and operating within Canada must follow the same procedures as Canadian businesses.

What we will ask when you order ROE forms

The person taking your order will ask for:

  • your business number —employer's number— issued by the Canada Revenue Agency (CRA),
  • the name, telephone number and address of your business.

If you do not have such a number, you will have to request it from CRAGovernment of Canada site before you place an order.  

If you plan on ordering in advance and picking up your ROE forms in person, you will  be asked to provide two pieces of identification, such as valid driver's license, photo identification card, major bank/trust company credit or banking cards.
