Application for a Permanent Resident Card (PR Card)

Initial Card, Replacement or Renewal

This application is for people with permanent resident status in Canada, who are applying for their first Permanent Resident card (PR Card) or for a renewal or replacement of a PR Card.

The PR card is a status document that provides secure proof of permanent resident status.

Document Checklist [IMM 5574]
Supplementary Identification [IMM 5455]
You will need an original Supplementary Identification form for each person applying for a PR Card.
Fees Receipt [IMM 5401]
You have two options to obtain a fee receipt.
1. Pay your fees on-line.
2. Pay your fees at a financial institution in Canada
  You will need an original fee receipt.

Complete our on-line order form to obtain the Supplementary Identification form, the fee receipt form or a complete application by mail.

If you are unable to or do not wish to use the on-line order form, you may also order by contacting our Call Centre.
