Tax Information Slips

This on-line service allows you to view and print copies of your Canada Pension Plan (CPP), Old Age Security (OAS) and Employment Insurance (EI) tax information slips. You can submit these copies with your Canada Revenue Agency (CRA) tax return.

In addition to viewing and printing your tax slips, you can also use the Tax Information Slips service to choose to stop receiving your tax slips by mail.

New for 2006! Employment Insurance (EI) recipients can now use this service.

Find the answers to several Frequently Asked Questions about CPP/OAS and EI on-line services.

To use this on-line service, you must:

  • have received a payment under either CPP, OAS or EI;
  • use a computer that meets our minimum computer requirements; and
  • login using either your Personal Access Code (PAC) for CPP/OAS programs or your EI Access Code.

Personal Access Code (PAC)

If you do not already have a PAC you can request one on-line. To login using your PAC you must also provide your social insurance number, first name, last name, date of birth and your mother's last name at her birth.

EI Access Code

Your EI Access Code is found at the bottom right hand corner of your first EI benefit statement. To login using your EI Access Code you must also provide your social insurance number, date of birth and province of residence.

Please note: You must provide either your 7-character PAC or your 4-digit EI Access Code each time you login. Keep your access code in a safe location and do not include it in any written correspondence.

Security Warning: If you are using this service on a shared computer in a public area (i.e. public kiosk or library), please do not leave the computer unattended while accessing this service. Before leaving the computer, it is also important that you fully log out of the application, clear your browser's cache and close down your browser. These precautions will ensure that no one else can access any personal information you may have entered.

Exceptions

  • For CPP - The Tax Information Slips service is not yet available for individuals between 18 and 25 years of age who receive CPP children's benefits.
  • For EI - If a separate tax slip is issued for benefits you received while participating in a Human Resources and Skills Development Canada employment measures activity you will not be able to view it using this service. This separate tax slip will be mailed to you.
  • If you have notified us that a registered trustee, guardian or other legal representative is taking care of your financial affairs, that individual is not authorized to use this service on your behalf. Your representative can conduct transactions on your behalf by mail, telephone or in-person.
  • If your CPP and/or OAS tax slips have been changed since originally issued in February, you cannot use this service to view the amended version of these tax slips. Use the amended copy mailed to you when you file your tax return with CRA.
  • Amended EI tax slips will be available on-line 4 to 6 weeks after the amendment is processed.
  • In very rare cases, you may be unable to login or view certain tax slips. This may happen if there is a discrepancy in how your information is stored on our system. If you are unable to access the service and are not subject to the other exceptions listed above, please contact us for assistance.
