Please accept our sincere condolences for the death of INSERT EMPLOYEE'S NAME.

The following information will help you understand your entitlements and benefits.

Payments are made to the estate or to a person entitled by law to share in INSERT EMPLOYEE'S NAME's estate and are subject to income tax withholding.  However, an amount up to $10,000 is tax free.

Supplementary Death Benefit (SDB)

This is a life insurance type benefit, equal to twice the annual rate of pay (rounded up to the next multiple of $1,000), paid to the estate or the person named by the deceased as their beneficiary.

As you are the designated beneficiary, you are entitled to $0000.00. This payment is a non-taxable benefit.

SDB Arrears

As INSERT EMPLOYEE'S NAME was on leave without pay, contributions for this coverage are outstanding in the amount of $0000.00.  This amount can be reimbursed by:

1.       cheque or money order made payable to the Receiver General for Canada for the full amount; 

2.       deduction of the full amount from any lump sum payments; or

3.       deduction from your monthly pension for a period of INSERT NUMBER OF MONTHS.

OR

Death Benefit

This is an entitlement representing two months of INSERT EMPLOYEE'S NAME's salary, paid to the surviving spouse or any other person as determined by Treasury Board of Canada Secretariat.

Payment for Month of Death

The estate is entitled to salary for the month of INSERT APPROPRIATE MONTH. The amount is $0000.00 (gross), covering the period from INSERT DATE to INSERT DATE.

As INSERT EMPLOYEE'S NAME was on leave without pay, two payments will be issued for salary for the month of INSERT MONTH OF DEATH:

First cheque (up to and including INSERT DATE OF DEATH) $000.00

Second cheque (after INSERT DATE OF DEATH)                  $000.00

Severance Pay

This is a lump sum payment of $0000.00 (gross) representing INSERT NUMBER weeks salary.

Leave Credits

The following earned but unused leave credits will be paid:

PENSIONS

Lock-in Provision

Employees are locked-in, as soon as they have accumulated two years of pensionable service.

Public Service Superannuation (PSSA)

You are entitled to receive a pension, payable from INSERT DATE OF DEATH based on INSERT NUMBER years of pensionable service.  The amount is estimated to be $000.00 gross per year or INSERT AMOUNT gross per month.

As INSERT EMPLOYEE'S NAME was making payments to buy back prior service, this service was automatically credited to his or her account with no further payments required. 

Children under 18 are entitled to a monthly pension.  Children between 18 and 25, who are full time students, may be entitled to a monthly pension.  They must have been a student since their 18th birthday or on INSERT DATE OF DEATH, whichever is later.

In order to receive their monthly pension, estimated to be $000.00 (gross), the enclosed forms"Declaration of Attendance at an Educational Institute" and "Statutory Declaration" are to be completed and returned to me for further action.

Please note that the above estimates are subject to verification by the Pension Administrator.

PSSA Arrears

As INSERT EMPLOYEE'S NAME was on leave without pay, pension contributions are outstanding in the amount of $0000.00.  This amount can be reimbursed by:

1.       cheque or money order made payable to the Receiver General for Canada for the full amount; 

2.       deduction of the full amount from any lump sum payments; or

3.       deduction from your monthly pension for a period of INSERT NUMBER OF MONTHS.

Monthly pensions are payable on the 3rd last banking day of each month.  A direct deposit service is offered providing the automatic deposit of monthly pensions to a financial institution. If you wish to take advantage of this service, you will need to complete the "Direct Deposit Enrolment Request" form and send it to me, along with a "voided" blank cheque. The first cheque will be issued as a supplementary payment and direct deposit should commence for the next cheque.

Indexing

The Public Service Superannuation Act and the Supplementary Retirement Benefits Act provide for annual increases to your pension, based on the cost of living index, and are effective on January 1st of each year.

Canada OR Québec Pension Plan

The Canada OR Québec Pension Plan provides survivor and death benefits.  Please contact the office nearest to you to obtain information and to apply for benefits.

Income Tax

Income tax (federal and provincial) will be deducted at source based on your province of residence (for non-residents, based on the country of residence).

Please complete the enclosed federal and provincial Personal Tax Credit  forms.

Enquiries should be made through Canada Revenue Agency and/or ministère du Revenu, Québec.

Public Service Health Care Plan (PSHCP)

You may apply for this coverage by completing the enclosed "Public Service Health Care Plan Pensioner Application"  form.  You will be assigned a new certificate number.  Deductions for this coverage are taken from your monthly pension and the rates are:

Dental Care Plan (DCP)

Coverage under this plan ceased on INSERT DATE OF DEATH.  However, certain dental treatments that began before INSERT DATE OF DEATH may continue to be covered if completed within 31 calendar days.  Please contact the Plan Administrator at:

Pensioners' Dental Services Plan (PDSP)

You may apply for this coverage by completing the enclosed Enrolment form. Once completed, it must be mailed directly to the address indicated on the form. If you enroll within 60 days of INSERT DATE OF DEATH, you will become a member from that date. If you choose not to apply within the initial 60-day period, you will still have the opportunity to join the PDSP at a later date.
