NominationsNominations are accepted through mail only, not on-line. To nominate someone, please complete a nomination form. Each nomination form requires specific information that you may add on a separate page or in a covering letter. There is no deadline for nominations for the Order of Canada, the Meritorious Service Decorations and the Governor General’s Caring Canadian Awards. However, nominations for the Bravery Decorations must be submitted within two years following the incident. Once completed, please send the form and attachments to:
