From the Investigator

The FTI shall summarise information contained in the preliminary report. The FTI shall describe factual information, immediate safety actions taken and the focus of the ongoing investigation. The FTI shall be distributed by mail to all flying units and headquarters organisations. The FTI shall also appear in the next quarterly issue of Flight Comment Magazine (and eventually on the internet). The FTI shall be published in bilingual format. The complete document shall not be longer than two (2) type-written pages.


Epilogue

The Epilogue shall summarise the information contained in the final report (FSIR). The Epilogue shall be distributed by mail to all flying units and headquarters organisations. The Epilogue shall also appear in the next quarterly issue Flight Comment Magazine (and eventually on the Internet). The Epilogue shall be published in bilingual format. The complete document shall not be longer than two (2) type-written pages, not including the title/picture page.


Flight Safety Investigation Report

Investigations of aircraft accidents and serious incidents are promulgated by means of the Flight Safety Investigation Report (FSIR). The report closely parallels the internationally accepted ICAO format. The report contains four sections: facts, analysis, conclusions and safety measures. The report is released to the public under the authority of the Director of Flight Safety, National Defence Headquarters, pursuant to powers delegated to him by the Minister of National Defence as the Airworthiness Investigative Authority (AIA) for the Canadian Forces.
