- Flight Safety
Information System (FSIS) Questionnaire : You
must complete the Flight Safety Information System (FSIS)
Pre-course
Assignment (link to 1 Cdn Air Div Intranet web site)
using the FSIS
Users Manual V3.0. It covers the basics of the software
used to input incidents and supplemental reports. The assignment
must be completed online prior to commencement of classes
on the first day. If unable to access Pre-course assignment,
please email Capt
Bob Schofield, Course Director.
- Flight Comment
Article : You are required to prepare a 200 -
400 word "anecdote-style" article suitable for use in "Flight
Comment" magazine. Your story should incorporate an aspect
of flying, maintenance, or your operation, that caused you
to reflect on the safety of your actions. The story must
be true but there is no need to embarrass yourself, individuals,
or other units so please use an appropriate writing style
. Ideally, the topic should have wide application throughout
the aviation community and be presented in a positive manner.
Please include your name, unit and phone number for editing
purposes. The best will be suitably reproduced in
"Flight Comment".
Please write the article in MS Word
2000 with the following name convention: LastName_TitleofArticle.doc
. The article shall be submitted to the
course director, Capt
Bob Schofield , via e-mail one week in advance of
the course.
- HFACS Reading
Assignement : A
reading
assignment entitled HUMAN FACTORS ANALYSIS AND CLASSIFICATION
SYSTEM (HFACS) by the US Navy must be completed before attending
BFSC.
