How to File a Report

As a health care provider who works with the WCB, you play a major role in the health and well being of the thousands of workers who are injured on the job in Alberta every year. Your role is essential in helping workers regain their quality of life.

Reporting to the WCB is a key component of the work you do with us. To make it easier we have introduced Online Electronic Injury Reporting.

  • Physicians can submit First Reports, Progress Reports and Invoices.
  • Chiropractors can submit First Reports, Progress Reports and Invoices.
  • Physical Therapists can submit Assessment Reports, Status Reports, Discharge Reports and Invoices.

There are many other WCB reports, which can be obtained online or by calling our Customer Contact Centre at 498-3999. There are three things you need to know about filing reports:

NEWS: You can now submit the Medical Care Invoice (C568) online. We request that all specialists use this form when invoicing the WCB. This invoice is also used by general practitioners who are billing for supplementary reports (WCB health service codes RF04 - RF08).
